I fully understand that this reaction can occur at any time even if I have received this service on previous occasions. I understand these risks and if I have any concerns, I will seek medical advice prior to any color/chemical service.Furthermore, I agree to follow the Quarantine Color Kit "Step by Step Instructions" provided in this document and assume all risks and hold Pistachio Hair Studio LLC harmless and not responsible for any damage resulting from the self-application of provided color/chemicals to my own hair including any possible adverse health reactions, property damage or other unforeseen problems.
